The novel coronavirus, also known as COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. As a result, federal, state, and local governments and federal and state health agencies recommend social distancing and have, in many locations, prohibited the congregation of groups of people. 

Grace Community Church has put in place preventative measures to reduce the spread of COVID-19; however, the church cannot guarantee that you or your child(ren) will not become infected with COVID-19. Further, based on the information provided by leading health officials, attending events such as worship services or children’s programs could increase your risk and your child(ren)’s risk of contracting COVID-19. 

By signing this Assumption of Risk and Waiver of Liability (“this Waiver”), you acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that you and/or your child(ren) may be exposed to or infected by COVID-19 by attending events at Grace Community Church and that such exposure or infection may result in personal injury, illness, permanent disability, and/or death. You understand that the risk of becoming exposed to or infected by COVID-19 at Grace Community Church events may result from the actions, omissions, or negligence of yourself and others, including, but not limited to, Grace Community Church employees, volunteers, and program participants and their families. 

By signing this Waiver, you voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to yourself and your child(ren) including, but not limited to, any personal injury, illness, disability, death, damage, loss, claim, liability, or expense of any kind (collectively, the Claims), that you or your child(ren) may experience or incur in connection with your/their attendance and participation in events at Grace Community Church, due in whole or in part to exposure to or infection by Coronavirus/COVID-19 or any variance.

On behalf of yourself and your children (if applicable), you hereby release, covenant not to sue, discharge, and hold harmless Grace Community Church, their employees, officers, agent representatives, and volunteers of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any claims based on the actions, omissions, or negligence of Grace Community Church, its employees, officers, agents, volunteers, and representatives, whether a COVID-19 infection occurs before, during, or after an event at Grace Community Church.

I (the undersigned) have read the above information, and I understand and agree that my signature represents a signature on behalf of myself and each of my children.
